The following forms are required by the Cherry Creek School district in order for your student to accompany the band on the bus to their events.

OHS Parent Consent and Insurance form - Print 2, fill out 2, and sign 2 forms
Consent for Excursion or Extended Trip Release - Print 1, fill out 1, and sign 1 form
Overland Conduct Policies and Procedures... - Print 2, fill out 2, and sign 2 forms
The following forms are required for expense reimbursement from the OHSBPA. The expenditure approval must be filled out and approved PRIOR to spending any money on behalf of the OHSBPA organization.
